Working closely with a dedicated team of physicians and clinicians,you’ll develop and manage care plans that support recovery and helppatients get back to the life they love. A s a Home Health LPN ,you will: Provide professional nursing services under the directionof a RN in compliance with the state’s Nurse Practice Act, anyapplicable licensure/certification requirement, applicablehealthcare standards, governmental laws and regulations, andCenterWell Home Health policies and procedures. Provide skillednursing interventions in the treatment of the patient/clientsillness, rehabilitative needs and preventative care. Utilize aholistic approach in the provision of problem specific therapeuticinterventions, teaching and training activities in accordance withthe plan of care. Apply knowledge and skills in accordance withaccepted standards of clinical practice to facilitate problemresolution and achieve individualized patient goals and outcomes.
Confer with the RN Supervisor regarding needed changes in the Planof Treatment. Accept verbal orders from physician were permitted bystate law/regulations/Nurse Practice Act and communicate theseorders to the RN Supervisor. Utilize a systematic, individualizedgoal/outcome driven approach in implementing the nursing plan ofcare. Maintain contact with patients, physicians, clinicalmanager(s), other members of the healthcare team in a timely mannerregarding patient/family needs and status changes. Participate incare coordination activities and discharge planning as appropriate.
Maintain the highest standards of professional conduct in relationto information that is confidential in nature. Share informationonly when the recipient’s right to access is clearly establishedand the sharing of such information is dearly in the best interestsof the patient. Appropriately communicate to ensure adherence toprofessional standards in the provision of and availability ofsupplies, materials and equipment needed to safely and effectivelyimplement the plan of care. Prepare, submit and maintaindocumentation as required by the Company and/or facility.

Required Experience/Skills:
Graduate of anaccredited Licensed Practical Nursing Program or accredited Schoolof Vocational Nursing. Current nursing license in the practicingstate. Valid drivers license, auto insurance and reliabletransportation. Current CPR certification. Two years experience asan LPN/LVN in a clinical setting, preferably in a home health orhospice setting. - $61,400 per year base pay Scheduled Weekly Hours 40 PayRange The compensation range below reflects a good faith estimateof starting base pay for full time (40 hours per week) employmentat the time of posting.
The pay range may be higher or lower basedon geographic location and individual pay will vary based ondemonstrated job related skills, knowledge, experience, education,certifications, etc. $44,600
