ResponsibilitiesJob Summary-The Home Care LicensedPractical Nurse is responsible for providing direct nursing care asprescribed by the physician to acutely ill, and/or chronically illpatients in their homes as well as recording clinical and progressnotes; and coordinating other patient care activities/services toprovide the highest quality of patient care in the geographicservice area to which she/he is assigned. She/he works under thedirect supervision of the Registered Nurse and/or designatedsupervisor. Nursing activities are limited to these in compliancewith the State LPN/LVN Practice Act, any applicablelicensure/certification requirements and agency policies andprocedures.
Specific Job Duties/Responsibilities Provides high quality clinical services within the scope ofpractice, as defined by the state laws governing the practice ofnursing, in accordance with the plan of care, and in coordinationwith other members of the health care team, following allapplicable infection control procedures. Documents patient visits thoroughly and completely perregulatory and payer requirements in the electronic medical recordutilizing the agency’s electronic visit verification program (asapplicable). Actively participates in the agency’s care coordination processincluding timely reporting patients’ needs and changes incondition, attending patient case conference, communicatingeffectively to the patient, family, physician, other clinicians,agency management staff, and case coordinator, community casemanagers (as applicable) Ensures confidentiality of patient health information peragency policyEducation And ExperienceLicense Requirements -Musthave current LPN licensure in state of Tennessee. Current driver’slicense, vehicle insurance and access to a dependable vehicle orpublic transportation. Current CPR certificationrequired.
Experience Requirements-Previous home care experiencepreferred.
