Position Summary
Under the direct supervision of the Director of Nursing and/or Assistant Director of Nursing, the Licensed Practical Nurse (LPN) coordinates and supervises the implementation of nursing care plans for assigned geriatric patients. The LPN assists the Registered Nurse, serves in a charge nurse capacity as needed, and performs semi-skilled nursing duties, including medication administration.
Key Responsibilities
1. Patient Care
Assist in assessing care needs and report observations to the RN.
Support care planning using Nursing Care Plans.
Deliver care related to activities of daily living (ADLs) and document in clear, concise nursing notes.
Provide therapeutic care and evaluate patient responses.
Contribute to short- and long-term goals, including discharge planning and patient/family education.
2. Medication Administration
Administer medications accurately, adhering to the “five rights” of medication delivery.
Follow all Department of Nursing policies and procedures.
3. Implementation of Care
Assist with ADLs and therapeutic care, including admissions and discharges.
Support physical exams and diagnostic procedures.
Collect specimens and provide postmortem care.
Deliver treatments (e.g., enemas, dressing changes, tube feeding).
Apply infection control and isolation procedures.
Initiate CPR and assist with emergency protocols.
Document patient responses to care plans thoroughly.
4. Evaluation of Care
Participate in care plan updates and revisions.
Engage in patient care and interdisciplinary team conferences.
5. Teaching and Training
Support and train other healthcare team members.
Participate in patient/family education.
6. Leadership and Coordination
Set care priorities and assign work to CNAs.
Serve as charge nurse when needed, overseeing nursing staff.
Assist with staffing and scheduling.
7. Professional Development
Engage in activities that improve patient care (e.g., QA initiatives, committees).
Attend a minimum of six hours of in-service training annually.
Participate in at least 75% of staff meetings.
Pursue continuous improvement and self-development.
8. Policy Compliance
Adhere to all Wesley Woods Senior Living policies and job expectations, including:

Bloodborne pathogen and isolation procedures
Qualifications
Minimum Qualifications
High school diploma or equivalent.
Current Practical Nurse license in the State of Georgia.
Basic Cardiac Life Support (BCLS) certification.
At least two (2) years of LPN experience.
Ability to lift and turn patients frequently, individually or with assistance.
Strong communication skills with patients, families, and team members.
Ability to stand or walk for up to 90% of the shift; frequent bending and stooping.
Sufficient visual acuity for chart review; adequate hearing and speech capabilities.
