Overview:

Under supervision of the registered nurse, the HospiceLPN performs a variety of patient care activities in the homesetting, or facility patient calls home. Provides care appropriateto the age of the patients served. Assists the registered nurse inassessing, planning, implementing, and evaluating outcomes of care.

Is accountable for own nursing actions and competencies.

Communicates effectively with others on the Interdisciplinary team.

Performs all duties in accordance with the State of Illinois NursePractice Act and established standards, policies, procedures andguidelines.

Qualifications:
Education:

Graduate of a state approvedschool of practical (vocational) nursing and currently licensed instate in which practicing

Licensure/Certification/Registry:

CurrentLPN licensure in the state of Illinois.

Current CPR certificationrequired.

Must possess a valid IL drivers license and must be deemedas an acceptable driver in accordance with the MHS fleet safetypolicy (five year MVR will be required).

AnnualCredentialing:

Attends all Joint Commission, OSHA and hospital/nursemanager specified mandatory in-services. Submits supportingdocumentation to the nurse manager in conjunction with the annualperformance appraisal.

Validates competencies and obtainscredentials as identified in unit specific requirements

Experience:

Aminimum of two (2) years clinical experience is required

OtherKnowledge/Skills/Abilities:

Must be able to position and/or transferan average adult patient and possess gross coordination sufficientto perform other patient care duties as required. Must be able toperform difficult manipulative skills.

Environmental workingconditions include exposure to blood, bodily fluids and tissue,contagious diseases and/or radioactive or other potentiallyhazardous materials.

Responsibilities:
Nursing CareData Collection:

Observes,records and reports objective and subjective patientdata.

Demonstrates ability to detect signs and symptoms of deviationfrom normal health status.

Informs RN of abnormal or changing dataand of patient/family responses to interventions.

Planning:

Assiststhe registered nurse in developing the nursing plan ofcare.

Provides input as applicable.

Intervention:

Completely performstasks/procedures as appropriately delegated.

Uses equipmentsafely.

Accurately administers medications by appropriatelydelegated routes.

Utilizes appropriate infection control safetymeasures.

Acts to prevent harm or injury to patient, self andothers.

Documents and communicates to RN nursing interventions andpatient responses.

Seeks supervision prior to initiating new orunfamiliar nursing activities and/or with a change in patientcondition.

Evaluation:

Observes, records and reports patientresponses to nursing interventions and other relevant data to theRN.Assists the registered nurse to evaluate and modify the plan ofcare.

Patient/Family Education:

Assists the registered nurse toidentify, implement and evaluate appropriate teachinginterventions.

Reinforces teaching initiated by other members of thehealthcare team.

Communication:

Informs RN of abnormal and/orchanging data and patient/family response to interventions.

Developsand maintains interviewing skills required for datacollection.

Medication administration:

May administer PO, injectable,topical, rectal, sublingual medications.

Wound CareQuality andSafetyCommunicates and documents potential risk issues toappropriate personnel.

Participates in continuous qualityimprovement.

Complies with all safetypolicies.
