- department. As an LPN in our facility, youwill play a vital role in providing high-quality nursing care toresidents who are undergoing rehabilitation or require long-termcare.

- Responsibilities
- Provide direct patient care, includingadministering medications, monitoring vital signs, and assistingwith daily activities.
- Support rehabilitation efforts by assistingpatients with mobility exercises, physical therapy, and otherrecovery activities.
- Collaborate with a multidisciplinary team,including physical therapists, occupational therapists, and otherhealthcare professionals, to create and implement individualizedcare plans.
- Monitor and document patient progress, including anychanges in condition, medications, and treatments.
- Educatepatients and families on care plans, medications, and lifestyleadjustments to promote recovery and well-being.
- Supervise andguide nursing assistants and other support staff to ensureconsistent, high-quality care.
- Advocate for patients needs,ensuring that their concerns are addressed and care plans arefollowed.
- Requirements
- Valid Licensed Practical Nurse (LPN)license in the state of practice.
- Current BLS certification
- Validdrivers license
