The nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies. Also, the RN/LPN role coordinates the total plan of care with other health care professionals, and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within the patients home. We are looking for compassionate nurses to provide skilled nursing services to patients in their homes in accordance with an established care plan!! Responsibilities Provide health care services in patients residences Follow physician prescribed patient care plan Administer prescribed medications Assist with clients personal care activities Complete & document patient assessment Assess, monitor, and document changes in patient symptoms or behavior Maintain patients care records and document provided services Provide companionship and basic emotional and/or psychological support Qualifications Trach/Vent experience preferred GJ tube experience preferred At 1 least one year clinical experience required Home health nursing experience required Must hold current licensure as a RN or LPN in the state of Georgia Strong problem solving and critical thinking skills Nursing and health care administration skills Caring and compassionate personality Ability to build rapport with patients & their family Incentives Weekly pay Flexible shifts to fit your lifestyle Employee referral bonuses for referring your friends and family
