Responsibilities:
Make outbound calls to patients scheduled for home visits and telehealth appointments Initiate chart preparation to assist providers with upcoming appointments Enter patient data into electronic medical records (EMR) systems Conduct phone-based assessments with patients
Required Software Skills:
Proficiency in Microsoft Office Suite Experience with eCW (preferred)
Working Hours:
Monday-Friday, 9:00 AM to 6:00
