The LicensedPractical Nurse is a proactive member of an interdisciplinary teamof licensed and unlicensed care givers who ensure that patients,families, and significant others receive individualized highquality, safe patient care. They practice in a clinical environmentthat is administered by Nurse Managers and other leaders andfunctions under the direction of the registered nurse through theWellStar Shared Leadership Model. It isexpected that all Licensed Practical Nurses are licensed,knowledgeable and uphold the practice of nursing as outlined by theGeorgia Professional Nurse Practice Act and implements the Scope ofPractice and Code of Ethics Standards put forth by the GeorgiaBoard of Nursing and the American Nurses Association. As a memberof the patient services team, it is expected that the individualupholds the voice of the patient, system policies and procedureswhile supporting service excellence goals.

It is expected that theLicensed Practical Nurse will not perform functions for which theyare not prepared by education and experience for which supervisionis not provided.

Core Responsibilities and EssentialFunctions:

Exemplary practiceParticipates in data collection forpatient assessment, planning, implementation, and evaluation of thedelivery of health care services and other specialized task whenappropriately educated under the supervision of the RegisteredNurse

  • Provides relationship-based patient centered care incollaboration with the registered nurse that is consistent withpopulation specific characteristics (e.g., age, gender, disease,etc..) in a manner that adapts service delivery to reflect anunderstanding of cultural diversity
  • Always partners with thepatient and significant others (as appropriate) using suchappropriate method for setting and purpose (e.g., bedside shiftreport; interdisciplinary rounds) keeping the patient andsignificant others updated and participating with the registerednurse in making the patients goals the focus of the plan of care
  • Practices using current clinical practice standards under thesupervision of the RN
  • Performs comfort and safety measuresincluding patient assistance with activities of daily living-Administers treatments and medications by various routes
  • Teamwork-Participates in the delivery and documentation of safe qualitypatient care under the supervision of the registered nursepromoting the professional care delivery model.

This includes butis not limited to diverse and inclusive interdisciplinarycommunication methods (interdisciplinary rounds, case review,etc..), completion of timely documentation and promotion of arespectful, inclusive clinical environment

  • Demonstrates teamworkand collaboration by practicing in a manner that is fiscallyresponsible and promotes the recruitment and retention of allhealthcare team members
  • Informs registered nurse of changes inpatients condition as appropriate.

  • Participates in theapplication of new practice changes under the supervision of theregistered nurse when called upon or through self-initiation incollaboration with care managers and other members of the healthcare team to achieve best practice outcomes (i.e., decrease inre-admission rates; avoidable days; adverse events; etc
  • Professional Development
  • Completes all initial and ongoingprofessional competency assessment, required mandatory education,and population specific education
  • Serves as a preceptor and/or ormentor for other licensed practical nurses.

Required Minimum Education:

Graduate of an accredited/approved school ofpractical/vocational nursing RequiredRequired Minimum License(s)and Certification(s):

Instructor Lic Practical NurseRequired Minimum Experience:

Two years of LPN experience in direct patient carepreferred
