Description As an
RN / LPN
with AccordCare, you will work to provide the highest quality of care to patients utilizing the Plan of Care provided to ensure patient’s needs are met in relation to safety, well-being and satisfaction. Experience with quadriplegic patients is highly preferred. Home care experience is highly preferred.
JOB RESPONSIBILITIES
Provides safe and effective care for the client based on individualized Plan of Care in conjunction with Clinical Manager.
Provides prompt communication of changes in patient medical status/emergencies to the team resulting in effective management of cases.
Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.
REQUIRED EXPERIENCE & EDUCATION
Licensure:
RN license or LPN license in the state of Georgia.
Experience with quadriplegic patients preferred.
Home care experience preferred.
Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
