The LicensedPractical Nurse (LPN/LPT/LVN) provides direct and indirect care toclients in their home environment in accordance with theorganizations policies and procedures. The LPN instructs,motivates, and assists clients in health maintenance and diseaseprevention to improve their ability to live independently at home.
The nurse directs the efforts of the Caregiver when involved in theclients plan of care. Based on state regulations, may performadministrative and/or field/clinical responsibilities whilereceiving oversight and clinical direction from a RegisteredNurse.

Provides skilled nursingcare/treatment to clients per Physicians orders and Plan ofcareAccurately documents nursing actions of all care given,individuals health status, and communicate with family andphysiciansDocuments the progress and outcomes for established goalsand informs physician, case manager, and families of changes inclient’s medical condition and needsProvides skilled nursingtreatments per physicians orders and within the scope ofpracticeMedication administration and management as neededProvideshealth teaching of client and assisting client in learningappropriate self-care techniques and safety measuresConductsnursing assessment visit as neededReports all incidents peroperation procedure and regulatory standardsFollows policies andprocedures according to operation, state and regulatorystandardsUses safe body mechanics to prevent personal injury toself or clientMaintains confidentiality of patient relatedinformation and honor patient rightsConsults, seeks advice, andfollows direction of the RN in fulfillment of care of theclientCompletes Start of Care assessments in person, whereapplicable and in accordance with state regulations (asapplicable)Completes supervisory visits based on compliance andregulations to include review of plan of care, caregiver competencyassessment, and completion of required documentation. Completesdaily, monthly, quarterly and annual nursing documents as/ifrequired.
Works closely with Director or Nursing/Nurse Supervisorand/or Branch Manager in planning, implementing, and evaluatingprograms and services to address the health needs of personsservedAttends and participates in staff meeting, in services, andother meetings as requestedEnsures compliance withstate/local/payer requirements and that documentation reflects thecare provided and meets company, regulatory, and payerstandardsEnsures the ordering, reviewing, monitoring,discontinuation and, if necessary, the disposal of medications asprescribed by the physicianTranscribes or ensures transcription ofphysician phone orders and other medically related reportsEnsuresthat furnishings, equipment, and supplies are maintained withinguidelines established to prevent the spread of diseaseEnsures thatappointments for health related issues are scheduled, completed andthat appropriate response to findings are implemented in a timelymannerCommunicates with Branch Manager, Customer Service Managerand/or Scheduler when clients are hospitalizedCommunicateseffectively and routinely with supervisor, ensuring that pertinentinformation reaches supervisor in timely mannerProvides newemployee training, annual re-certification training, and competencyvalidation for health and safety related topicsMay be asked tocover hours, weekends, and holiday on call to meet the needs ofclientsOther duties as assigned
Qualifications:
Degree in Nursingfrom an accredited schoolExperience in administering medicationsOneor more years of nursing experience preferredHome health careexperience preferredCurrent and unrestricted license in the stateof the operationCPR professional certificationBasic clinicalskillsProfessional appearance and demeanorGeneral knowledge ofcomputers and the ability to learn industry specific software anddocumentation applicationsExcellent oral, written, andinterpersonal communication skills with all levels of personnel,internal and external to the companyExcellent organizational andtime management skills with a keen attention to detailsCapable ofworking responsibly with highly confidential informationAbility towork effectively independently or in a team environmentAbility todevelop critical thinking skills, research situations, solvecomplex problems and deal with a variety of issuesAbility to learnand complete various educational and position specific goals
