Position SummaryAre you aLPN who is looking to expand your skills with nursingadministration in the home healthcare field? This position willrequire the LPN to travel to our patients’ home to completesupervisory visits with our clients receiving personal careassistance.

Responsibilities Conduct Initial and Annual Skills Training of PCAs. Re-evaluation of patients’ skilled nursing/PSS needs Work autonomously with minimal assistance by the ClinicalManager Maintain patient confidentiality Make decisions based on established policies andprocedures Preparation of the clinical progress notes. Informing the family and Registered Nurse of changes in thepatients’ condition or needs. Supervisory Visits and teaching in Patient’s Home. Administration of medications and treatments as prescribed by aphysician in accordance with currently accepted standards ofmedical practice Total patient care Patient and family teaching. Other services in accordance with and as outlined in theGeorgia Licensed Professional Nurse Practice Act. Reports directly to the Registered Nurse and ClinicalDirector.

Qualifications Licensed Practical Nurse (LPN)) must hold a currentunencumbered Nursing license to practice in the state ofGeorgia. LPN will complete the assigned onboarding nursing quiz. LPN should have experience caring for medically fragileindividuals. It is preferred that the LPN have experience in the home healthservice field. Current BLS Certification.
