WORK SCHEDULE
Monday through Friday 9:00 am – 5:00 pm. In office (4 days) and virtual (1 day) per week.

QUALIFICATIONS LPN
License from State of Georgia Education verification and other credentialing based on position requirements. Must pass a national background check and fingerprinting. Valid Georgia driver’s license and satisfactory driving record (MVR). First Aid/CPR AED certifications. Provide negative results Tuberculosis screen.
RESPONSIBILITIES
Demonstrate knowledge of the needs of people with I/DD. Provide nursing and wellness coordination. Schedule appointments for clients who have intensive support needs. Utilize the MyChart Patient Portal at Emory Healthcare to access records. Participate in pharmacy/medication reviews. Be proficient at using Therap Integrated Health Management System. Document health and care needs on the electronic health record. Maintain accurate and complete electronic health records. Participate in and help guide weekly healthcare meetings. Perform nursing assessments. Perform nursing supervisory visits. Update the Health Risk Screening Tool (HRST). Demonstrate strong documentation management skills. Develop and/or adjust individualized care plans and protocols. Suggest and coordinate followup services to promote wellness. Train direct care staff on healthrelated policies and procedures. Train direct care staff on assisting with medications. Train direct care staff on implementing healthcare plans.
PHYSICAL REQUIREMENTS / WORK ENVIRONMENT
This job operates in an office setting. While performing the duties of the job, the employee is required to frequently stand, walk, sit, stoop, reach, bend, twist, kneel, push, and pull for periods of time as well as see, talk, and hear. The employee may frequently move to interact with fellow employees, visitors, and/or clients. Additionally, the employee may be required to lift objects weighing up to 50 pounds.
