Job Description Position Type:
Full-Time, Salaried Key Responsibilities:
Coordinate and support all field contracted staff to ensure timely completion of necessary assessments. Conduct assessments of clients’ suitability for services. Perform administrative tasks to support the smooth operation of client services. Travel to clients’ homes to conduct face-to-face assessments as needed. Work alongside the RN Supervisor to ensure the department is in compliance and clients have the proper care they need.
Qualifications Required:
Active LPN License Reliable transportation to travel within the service area
Preferred:
Previous experience in a similar role Strong administrative and computer skills Excellent organizational and time-management abilities
