Jelly Nurse Responsibilities Provides nursing care to patients and as delegated by assigned supervisor and/or provider in order to meet the needs of the patient/family Collects data and information to assist the clinical team and/or the provider in patient assessment and care planning which includes but is not limited to: 1. Triaging patients telephonically Acts as an EMR support liaison and maintains communication logs Providing support for non-urgent patient education(e.g., heart failure diets, device instructions, etc) 2. Communicating internally & externally through telephone encounters & actions Medication Refills Follow Up Needs Normal Result Reporting 3. Reviewing documents Reviewing provider schedule and prepping charts in advance of visits Tracking incoming consult reports, imaging, and lab results for completion 4. Reviewing hospital/clinical records 5. Prioritizing tasks Completing patient assistance, all FMLA, disability paperwork, prior authorizations, insurance appeals, etc and communicating with patients Delivers care in accordance with the patient care plan, as well as in accordance with policies and procedures of the office Participates in teamwork Supports midlevels and physicians with documentation needs; to include, order entry, letters, and referrals Effectively and respectfully communicates with other departments of the office Demonstrates cardiac related knowledge and appropriately applies knowledge to patient care Assisting in closing care gaps and ensuring following up for abnormal findings Assisting in the onboarding of new patients Demonstrate excellent communication skills (verbal and written) as well as computer/electronic medical record systems skills Exhibit an ability to adapt to unpredictable situations within the work setting Monitoring clinic pace and flow anticipating delays or issues and assisting where needed Independently completes assignments in a timely manner Triage Nurse Responsibilities Collects data and information to assist the clinical team and/or the provider in patient assessment and care planning in accordance with current MIPS guidelines which includes but is not limited to: Triaging patients Vital Signs ECGs Weight Updating patient history and medication list Performing real-time clinical assessments and EMR Documentation during visits Supporting providers with in-room needs Vitals, ECGs, patient positioning, and assisting with procedures Escorts patients to and from rooms and assist with mobility as needed Preparing and stocking exam rooms before and during clinic as needed Assisting with emergency responses (e.g. calling 911, using AED) Monitoring clinic pace and communicating delays or issues to the care team/ nurses station Other duties as assigned
Requirements:
Education and Qualifications Holds/maintains active CMA license or Holds/maintains active LPN license, active in Georgia Holds/maintains a current MA certification and maintain Basic Life Support (BLS) Minimum 1 year of healthcare experience, preferably in outpatient setting with working cardiac knowledge preferred Corporate Culture Expectations Demonstrates the office Corporate Culture at all times with colleagues, providers, outside offices/vendors, patients and their family members.
