The private duty nurse provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies. The private duty nursing role coordinates the total plan of care with other health care professionals, and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within the patients home. We are looking for compassionate nurses to provide skilled nursing services to patients in their homes in accordance with an established care plan!! Responsibilities Provide health care services in patients residences: trach, g-tube, wound, hygiene Follow physician ordered patient care plan Administer prescribed medications Assist with clients personal care activities Complete & document patient assessment Assess, monitor, and document changes in patient symptoms or behavior Maintain patients care records and document provided services Provide companionship and basic emotional and/or psychological support
