Job Summary:

The Resident Care Coordinator is a licensed professional employee (LPN) trained to supervise personal care and related services for the residents in the community. He/she functions under the Wellness Director and/or Executive Director. Essential Functions and responsibilities may include, but are not limited to the following : 1. Responsible for the scheduling of staff, including monitoring coverage and over time. 2. Participating in the manager on duty program. 3. Managing medication carts on all floors to ensure compliance with regulations. 4. Direct supervisor to all clinical staff, including enforcing disciplinary action when needed, as directed by the Wellness Director and/or Executive Director. 5. Monitoring care staff documentation to ensure compliance with state regulations and company policy. 6. Assisting Wellness Director with managing the care of all residents within the community by ensuring all care plans are implemented as needed, including the communication of care to family members. 7. Train and educate care staff as needed, including new hires, in services, and situational training. 8. Filling in for care staff when needed as directed by Wellness Director and/or Executive Director.

Education and Experience :

LPN with at least 2 years of experience in a long term care setting. Experience dispensing and monitoring medications on time. Experience managing certified medication aides and certified nursing assistants to ensure compliance with regulations.

Physical Demands and Working Conditions:

Must be able to lift 50 lbs. Must be able to sit, stand and walk for extended periods of time. Must have flexibility with schedule and availability to work weekends.

Management/Decision Making:

Direct supervisor to staff and daily assignments.

Knowledge and Skills :

Must display sincere interest in caring for the elderly. Must possess and openness in maintaining a multi-disciplined team approach to caring for our residents. Must be a team player and willing to have a positive working relationship with all associates. Must be organized, assertive, demonstrate critical thinking skills and be an effective communicator. CPR and first aid certified. Reports to : Resident Wellness Director and/or

$55,000.00 – $60,000.00 per year
