Overview:

The LPN will serve as amember of the Enact Transitional Homes team and is responsible forhealth and safety of individuals and to coordinate and providetheir services. The Transitional Home is used as a measure of lastresort for individuals in inpatient hospital facilities with noother community placement options. The goal is to stabilize theindividual through nursing and behavioral supports while preparingfor a permanent placement.

Responsibilities:

Work with the treatmentteam to provide direct care to individuals residing in TransitionalHomebased upon the individual treatment plan.

Care includesproviding medical treatments, administering medication and givinginjections.

Ensures that an inventory of medication is safely andsecurely maintained.

Helps individuals with all activities of dailyliving.

Ensures healthy meals and snacks are provided whilefollowing menu plans.

Measures and records individuals vital signs,such as height, weight, temperature, blood pressure, pulse andrespiration, blood sugars etc.

Documents and reports any changes inindividuals progress or conditions. Uses positive behavioralsupport strategies as described in behavior support plans.

Assistwith the implementation of high intensity services such asscreening, evaluation andassessment within the limits of theirlicense.

Facilitate individual transition to other supports and/ortreatment. Comply with all standards to assure the health andsafety of all staff and individuals that we serve. Must report anysuspected abuse, neglect or exploitation to supervisor ordepartment head. Attend all scheduled training and staffmeetings.

Plan and participate directly in recreational,therapeutic, and training activities of theindividuals.

Qualifications:

Valid unrestricted Georgia NursingLicenseCertification and ongoing training in crisis interventioncurriculum.

Valid CPR and First Aid CertificationExperience workingwith individuals with intellectual and/or developmentaldisabilities and/or mental illness who are in crisis.

Knowledge ofDBHDD community provider standards and policiesValid DriversLicense and auto insuranceMinimum of year supervisoryexperienceMust be computer literateBenchmark Human Services is anEOE/AAP Employer.
