Overview:

The LPN willserve as a member of the Enact Transitional Homes team and isresponsible for health and safety of individuals and to coordinateand provide their services. The Transitional Home is used as ameasure of last resort for individuals in inpatient hospitalfacilities with no other community placement options. The goal isto stabilize the individual through nursing and behavioral supportswhile preparing for a permanent placement.

Responsibilities:

Workwith the treatment team to provide direct care to individualsresiding in Transitional Home based upon the individual treatmentplan. Care includes providing medical treatments, administeringmedication and giving injections. Ensures that an inventory ofmedication is safely and securely maintained. Helps individualswith all activities of daily living. Ensures healthy meals andsnacks are provided while following menu plans. Measures andrecords individual’s vital signs, such as height, weight,temperature, blood pressure, pulse and respiration, blood sugarsetc. Documents and reports any changes in individual’s progress orconditions. Uses positive behavioral support strategies asdescribed in behavior support plans. Assist with the implementationof high intensity services such as screening, evaluation andassessment within the limits of their license. Facilitateindividual transition to other supports and/or treatment. Complywith all standards to assure the health and safety of all staff andindividuals that we serve. Must report any suspected abuse, neglector exploitation to supervisor or department head. Attend allscheduled training and staff meetings. Plan and participatedirectly in recreational, therapeutic, and training activities ofthe individuals.

Qualifications:

Valid unrestricted Georgia NursingLicense Certification and ongoing training in crisis interventioncurriculum. Valid CPR and First Aid Certification Experienceworking with individuals with intellectual and/or developmentaldisabilities and/or mental illness who are in crisis. Knowledge ofDBHDD community provider standards and policies Valid Driver’sLicense and auto insurance Minimum of year supervisory experienceMust be computer literate Benchmark Human Services is an EOE/AAPEmployer.
