LPN Hospice Nurse Position Available In Ascension, Louisiana

LPN Hospice Nurse Hospice in His Care Ascension Parish, LA Hospice in His Care is a locally owned and operated company that has been serving our Baton Rouge community for many years. We are looking to add an LPN to service our patients in Ascension and surrounding areas. Flexible schedule and hours with competitive pay. Come join our amazing team!

Duties:

• Provide direct and indirect patient care in a variety of healthcare settings
• Perform assessments, develop care plans, and implement nursing interventions
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report any changes or abnormalities
• Provide education and support to patients and their families on various healthcare topics
• Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
• Document patient care activities accurately and timely

Experience:

• Minimum of 2 years of experience as a licensed Nurse
• Experience in Hospice
• Proficient in planning and coordination of care for patients transitioning in home or other healthcare facilities

Requirements:

• Valid Nurse license in LA
• Current CPR certification
• Strong communication and interpersonal skills
• Ability to prioritize and multitask in a fast-paced environment
