The LPNfunctions as a member of the multidisciplinary health care team,and utilizes the nursing process in implementation of patient’scare. Those services are performed under the supervision of aregistered nurse but utilize all nursing skills to make safe,appropriate and independent decisions within the scope of the LPNnursing practice. All duties to be done in accordance with JointCommission, Federal and State regulations, Oceans’ Mission,policies and procedures and Performance ImprovementStandards.
Essential Functions:
Delivers quality nursing care toassigned group of patients by collecting data and identifyingdeviations from normal; assists with reassessing patients accordingto assignments, need and unit protocol and/or hospital policy underthe direction of the registered nurse.
Accurately and thoroughlydocuments all findings in the patient record and identifiesexpected outcomes based on assessment data; effectivelycommunicates pertinent assessment findings with other health caremembers.
Implements nursing care interventions identified in theindividualized multidisciplinary treatment plan using acceptedstandards of practice in collaboration with other health careproviders; performs nursing procedures/treatments as prescribed byphysician/LIP.Conducts specified bedside laboratory tests, woundcare, and provides for comfort and assistance in the performance ofADLs;Promotes the patient’s ability to cope, adapt and/or solvesituations related to illness and/or stressors.
Provides supportive,age appropriate communication and culturally acceptableinterventions and documents special needs.
Provides for the safedelivery of medications in accordance with the facility policiesand physician orders; administers medications utilizing establishedsafety procedures / standards. Ensures patients are effectivelyassessed prior to and after medication administration. Accuratelycommunicates/documents medication doses prescribed, and providespatient education on medication administered.
Follows up on patientcare responsibilities; communicates incomplete assignments andpatient needs with other disciplines to assure continuity of care;transcribes orders accurately and performs chart audits oftranscription accurately and according to facilityguidelines.
Utilizes nursing knowledge to successfully meet theneeds of the facility, patient population and staff members.
Utilizes nursing resources to enhance evidence-basedpractice.
Guides, educates, and provides leadership as a preceptorto new staff. Performs other duties and projects asassigned.
RequirementsCurrent, valid, and active license to practiceas a Licensed Vocational/Practical Nurse in the state of which youare employed at an Oceans Healthcare hospital. Valid Compactlicenses are also acceptable as they apply. High School Diploma orGED is required. One year of inpatient hospital experience stronglypreferred.
Must be able to work in high stress and assess potentialfor patient escalation. The LVN/LPN must be able to communicateeffectively both orally and in writing, to consistently achievepositive results with internal and external customers, be highlyorganized, have an understanding and sensitivity to serveculturally diverse and special needs population such as ChronicallyMentally Ill, Geriatric, CD, as well as be versed in basicknowledge of normal physical and emotional development. Exhibitscritical thinking abilities and applies them for continuousimprovement of patient care. Demonstrates self-confidence andability to think conceptually in leading and directing others. Mustsuccessfully complete CPR certification and an Oceans approvedbehavioral health de-escalation program.
Subject to manyinterruptions. Occasional pressure due to multiple calls andinquiries. This position can be high paced and stressful; must beable to cope mentally and physically to atmosphere. This personmight sometimes experience disagreeable odors, sights and/orunpredictable patient behavior. Work requires spendingapproximately 90% or more of the time inside a building that offersprotection from weather conditions but not necessarily fromtemperature changes.by Jobble
