Job Duties:
Provide nursing and supportive carein accordance with the Interdisciplinary Plan of Care, under thedirection of the RNCommunicate with the attending physicians, otherhome health team members, and other agencies as needed tocoordinate optimal care and use of community resources for thepatient/family.
Document all services provided.
