Mid-City Community Nursing and Rehab is hiring for anexperienced Full-Time MDS Coordinator RN/LPN!

• $2,000 Sign OnBonus
• The MDS Coordinator RN/LPN supervises and leads UnitSecretaries, Certified Nurse Assistants & License Practical Nursesin providing quality of care for patients of all age groups byperforming basic nursing tasks. Facilitates workflow practices byproviding support services as required by the nursing staff.
• NoOn-Call required

Duties:

Completes SNF MDS assessments and transmitsaccordingly.

Oversees assessment processes, set assessmentschedules, and assures assessments are done in an accurate andtimely mannerParticipates in planning for discharge.

Observes andreports responses to interventions as assignedCommunicates withpatient, family, caregivers, significant other, and members of thehealthcare team to promote maximum benefit of care.

Completes alldocumentation required to support the services provided and meetorganizational, clinical and reimbursement requirements.

Followsinfection control guidelines as per nursing policy andprocedureResponds to department needs. Participates in andcontributes to the quality improvement process for the departmentand the institution.

Qualifications:
