$51.2K•$62.8K a year 15 hours ago Qualifications Program management Mid-level 3 years LPN Associate’s degree Care plans Nursing Full Job Description The LPN Medicare Nurse is responsible for the co-ordination of the admission through discharge processes of the Medicare Part Skilled Unit patients. Program Management and Daily Operations Daily a.m. rounds on SNF residents to check: change in condition. Daily checking of charting. Attending weekly Medicare meetings Making sure that certifications and re-certifications are signed Weekly assessments on each SNF resident with documentation in nurses’ notes in EMR system. Assist with coding of bills Admission of new residents if applicable and when possible Creating MDS on each resident and updating MDS as required (5 day, 14 day, 30 day, etc.) Ascertaining that weekly/monthly weights are done and charted. Addressing significant weights loss/gain with family and physician. Implementing Plan of Care on each resident. Keeping logs and forms that may be specific for Medicare Part A. Drug Administration Functions Review charts of Skilled nursing patients for completeness of MAR’s Assists the Director Nurses in maintaining compliance with all DHH/Federal Regulations. Make sure that medications for skilled residents are ordered promptly and are a 14-day supply. Charting and Documentation A complete admission assessment record on new admits when possible Chart weekly nurses’ notes in an informative and descriptive manner that reflects the care provided and the resident’s response to the care. Review new/changed orders. Complete and update the MDS on each resident and make sure that it is transmitted to State in a timely manner Ascertain that discharge summary is completed when a resident is discharged Perform any charting in accordance with charting and documentation policies and procedures. Nursing Care Functions Greet newly admitted residents upon admission. Participate in orientation of new residents/family members to the facility. Consult with the resident’s physician in providing skilled resident’s care, treatment, rehabilitation, etc. Provide any professional services as in the scope of practice of a licensed practical nurse as required or indicated. Ensure that staffs taking care of skilled residents are providing care according to resident’s care plan. Ensure that residents who are skilled and who are unable to call for help are checked frequently. 3 years clinical experience as a Licensed Practical Nurse Diploma or Associate’s Degree accredited LPN program CPR ;Licensed Practical Nurses
