St. Joseph Hospice – New Orleans, LA is seeking PRN On-Call Licensed Practical Nurses (LPN) to join our patient care team. Essential duties include:
Provides nursing and supportive care in accordance with Interdisciplinary Plan of Care, under the direction of the RN.
Communicates with the attending physicians, other hospice team members and other agencies as needed to coordinate optimal care and use of community resources for the patient/family
Document all services provided and submit on appropriate forms.
Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
Report changes in patient condition and needs to RN
Great time management skills
Qualifications:
Current Licensed Practical Nurse (LPN) in the State of Louisiana
Minimum of 3 years as a Licensed Practical Nurse (LPN) of varied work experience, two (2) years in acute care setting and one (1) year in home care or hospice preferred.
Have dependable transportation, driver’s license and car insurance
Previous Hospice or Home Health experience required
