Full Job Description General Description:
The Licensed Practical Nurse participates as a member of the health team in administering nursing care at a community-based health center. Under the direction of the Nursing Supervisor, the Licensed Practical Nurse (LPN), assists in assessing patient needs, providing treatment, and initiating referral and follow-up services indicated for total patient care.
Duties:
The person filling the Licensed Practical Nurse position will have the following duties: Must be willing and able to assume the responsibilities assigned to her as a member of the health team to provide safe, efficient and effective nursing. Must be able to adapt to changing situations Willingness and ability to learn or develop technical skills necessary in providing health. Responsibilities:
The Licensed Practical Nurse (LPN), under the direction of the Nursing Supervisor or her designee within the guidelines of the program, assists in assessing patient needs and providing the following: Perform “triage” in the clinic for “walkin” patients to assist the Nursing Supervisor in determining and meeting patients’ immediate health. Participate in administering first aid measures and lifesaving procedures according to guidelines to control or reduce danger to the patient. Participate in the collection of necessary information concerning the patient to aid the physician in providing medical care. Participate in assisting the physician and the Nursing Supervisor with necessary procedures and examinations; give medications and carry out assigned duties in providing total patient care. Participate in interpretation of physician’s instructions and orders to the patient and/or the family to meet immediate health needs. Participate in health education and orientation to improve the general wellbeing of the family. Participate in arrangements for followup care related to immediate needs. Participate in initiating referrals to other departments in the clinic when indicated to meet additional needs. Participate in preparation and maintenance of clerical equipment, supplies and environment for general operation as outlined by the program. Participate in preparing and giving immunization as required by guidelines. perform other duties as required by supervisor/clinic administrator.
Qualifications:
Valid LPN license Proficient inpatient care and assessment Experience with triage Knowledge of EMR and EHR systems Basic life support certification Familiarity with HIPAA regulations Experience in critical care, longterm care, or assisted living settings Strong understanding of physiology and tube feeding techniques Ability to work effectively with diverse patient populations Expected hours: 40 per week
