• Petinsurance As a Licensed Practical Nurse Clinical Coordinator,you’ll contribute to our success in the following ways:
• PromotesElara Caring’s philosophy, mission statement and administrativepolicies to ensure quality of care.
• Reviews and clears HCHBcoordination notes and processes administrative task workflows inan accurate and timely manner
• Obtains orders from physicians,including verbal, written and faxed orders, and enters intoappropriate system for CTM review and approval.
• Reviews, updates,and maintains the At-Risk registry.
• Provides clerical support topatients such as but not limited to scheduling follow-upappointments for patients (as requested), providing healthsummaries to case managers, obtaining pre-authorizations, andaddressing patient questions or concerns.
• Performs chart auditsas required by Elara Caring policy.
• Verifies receipt of notice ofMedicare provider non-coverage to patient.
• Maintains patient andstaff privacy and confidentiality pursuant to HIPAA Privacy FinalRule.
• Bilingual Spanish/English is a plus
• May perform fieldvisits (direct care), as needed
• Performs other duties/projects asassigned. What is Required?
• Graduate of an accredited school ofprofessional nursing.
• Current, unrestricted LPN license valid forthe state of work
• Minimum 1 year of experience as a nurse in aclinical setting.
• This is an in-office position but need to beable and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and currentauto insurance in accordance with state laws.
• Ability to sit,stand, bend, lift and move intermittently and be able to lift50-100 lbs.
