Job Description
The LPN triages patients by telephone or walk-in to determine theurgency and level of care needed. The LPN is responsible for assistingthe Practice Manager in the supervision of clinical support staffworking within the office and for assisting in patient care under theguidance of CHP Physicians. Duties will be performed as assigned andallowed under their license. This person will support CHP’s mission,vision, and values and will adhere to compliance protocols as well asCHP’s policies and procedures.

Licensed Practical Nurse EssentialDuties and Responsibilities:
- Triage patients both by telephone orwalk-in; approve same day visits when appropriate; assist with referralsto area specialists; PT-1 forms; patient education; supervising medicalassistants as needed.
- Assist physicians in all procedures requiringnursing level support; administers immunizations as well as allergyshots; manage requests for prescription medication; process laboratoryresults, pull charts, review and hold partials
- flagging abnormal forprovider review and filing normal; contacting patients with results asdirected; chart all procedures performed.
- Assist with running dailycontrols on equipment; assist in charting for quality assurance (HCG,strep, sickle cell, urinalysis dipstick, etc.); ensures that billingcodes are correct; other routine office duties as requested.

- Spanish speaking/bilingualis a plus.

Education:
- This position requires graduation of anaccredited school of practical nursing.

Experience:
- A minimum ofthree years’ experience in an ambulatory setting is preferred.
Electronic medical records experience is required. CPR certification isa plus.
- Required License(s) and/or Certification(s):
- Must belicensed as a practical nurse by the Commonwealth of Massachusetts.
