• Essential Job Functions:

• LPN provides basic nursing care suchas checking blood pressure, inserting catheters, and helping comfortpatients by performing tasks such as changing and dressing
• Documentaccurate and ongoing assessment of patient status: Document patientcare, including nursing intervention, patient response to care provided,patient needs, problems, capabilities, limitations, and progress towardgoals
• LPN communicates with RNs and physicians regarding patients?needs
• Collaborate with other nurses and health team members to ensurepatients? wellbeing
• Teach patient/significant other appropriate healthinformation in a timely manner and share written informational material,as applicable.
• Perform direct patient care, using establishedprocedures, policies, and standards
• Prepare and administer medicationsaccording to policy and procedure.
• Observe and document patients?responses to pertinent medications.
• Demonstrate ability to handleemergency situations in a prompt, precise, and professional manner.
• Admit, transfer, and discharge residents as required.
• LPNs performadministrative duties such as completing medical forms, reports,evaluations, charting, etc., as necessary.
• Receive phone orders fromphysicians and record on the Physician?s Order Form.
• LPNs chart allreports of accidents/incidents involving residents. Follow establishedprocedures.
• Verify the identity of each resident before administeringthe medication/treatment.
• Ensure adequate medications, supplies, andequipment are available. Report needs to DON.
• LPNs ensure narcoticrecords are accurate for your shift.
• Notify DON of all drug andnarcotic discrepancies.
• Give a thorough, concise report at change ofshift. Communicate pertinent information to the appropriate person in atimely manner.
• LPNs perform all job responsibilities in accordancewith safety and infection control policies and procedures, includingthorough hand washing, use of disposable gloves where indicated andproper disposal of soiled materials.
