Responsibilities:
The LPN MDS Coordinator participates in the Resident AssessmentProcess through the completion of pertinent items on the MinimumData Set (MDS) and the accompanying Care Area Assessments (CAAs) toinsure the appropriate patient care and nursing centerreimbursement for the care provided.
- Direct the nursing centerinterdisciplinary Resident Assessment Instrument (RAI) processwhile following guidelines for completion, computer encoding andelectronic State transmission of the MDS.
- Participate indeveloping individualized resident Care Plans, identifying theneeds of the resident and projected outcomes; as required byFederal and State regulations.
- Train new members of theinterdisciplinary team on clinical content and electroniccompletion of the applicable MDS items.
Qualifications:
- Must bean LPN in good standing and currently licensed by the state.
- Previous experience in a hospital, long-term facility, or otherhealthcare related facility is required.
- Must be knowledgeable ofgeneral, rehabilitative and restorative nursing and medicalpractices and procedures and laws, regulations and guidelinesgoverning long-term care.
- Must have computer skills to includewindows applications. AccuMed experience is preferred. Previousexperience as an MDS Coordinator is preferred.
- Prior experiencein completing MDSs and Care Plans is required.
- Demonstratedproficiency in RAI/PPS processes is required.
- Must maintaincurrent BLS/CPR certificate (excluding ALFs/ILFs).
