Residential LPN Care Coordinator Job Summary:
Coordinates all aspects of health care for individual served in collaboration with the treatment team to ensure maximum level of health and wellness. Assesses, plans, implements, coordinates, and evaluates options and services to meet an individual’s health care needs through communication and available resources to promote quality, cost-effective outcomes
Case Management Responsibilities:
- Conducts a comprehensive nursing assessment of the individual’s health and psychosocial needs and develops a Case Management Plan collaboratively with the individual, family and caregivers.
- In conjunction with The Program Director, develops and maintains Health Care Plan (Section 2a: Medical Plan and Section 2b Nursing Care) for each individual supported in the Residential Division.
- Reviews, modifies and updates the Health History biannually, consistent with the ISP and Annual Physical.
- Completes DDS Health Care Record and Health and Dental within ISP timelines.
- Completes HICIS documenting Hospital admissions and discharges.
- Reconciles medication whenever a transfer occurs.
Major Responsibilities:
- Accompanies individuals to doctor’s appointments and advocates for them while at appointment.
- Utilizes a PBS approach when working with staff and individuals, including promoting the agency objectives related to personal growth, community involvement and decision making
- Reports clinical change in status to RN case manager or On-Call RN.
- Acts as an advocate and liaison for the individual to assure that they are receiving a level of care consistent with clinical best practices.
Education and Experience:
Must be fully licensed as a Licensed Practical Nurse in Massachusetts and must maintain licensure from the Massachusetts Board of Registration in Nursing. Minimum two years nursing experience in the direct care of the elderly or disabled adults is required.

Must drive to various locations for home visiting purposes and medical appointments. Light physical effort might be required. Ability to lift or pull up to 40 pounds. Able to use Hoyer lift to transfer individuals. Able to shower individuals using shower chair. Maintain a safe work environment by following

Monday through Friday 7:30 am to 3:30 pm
