Videos Duties This Licensed Practical Nurse (LPN) position is loced under the Primary Care service line within the Central Western Massachusetts Healthcare System the Greenfield, MA locion. The LPN works under the direction of a Registered Nurse and reports to the Primary Care Nurse Manager. This position is responsible for observing, recording and reporting changes in the pient’s condition, as well as assisting in the implemention of the pient-driven holistic care plan for life. Major duties include but are not limited to: Provides support for the Pient Aligned Care Team in the Primary Care clinics. Participes in selected health screenings, clinical reminders, telephone calls, and nursing trements. Administers and documents medicions. Draws, centrifuges and packages lab samples. Ensures th all outpient clinic appointments are scheduled in accordance with the business rules, policies, and procedures. Supports a pient focused environment through courteous and respectful dealings with pients, families, and coworkers. Actively participes in the development and implemention of quality improvement initiives. Participes and contributes to interdisciplinary teams for resolving pient care and system issues.

English Language Proficiency :

Licensed practical or vocional nurses appointed to direct pient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Grade Determinions :

GS-3 None beyond the basic requirements. LPNs this grade level are expected to perform all duties within the scope of their license (e.g., medicion administrion); however, completed work may require routine or detailed higher-level review depending upon the complexity of the duties involved. GS-4 Six months of qualifying experience as an LPN or LVN; OR Graduion from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience th involved nursing care work in a hospital, outpient clinic, nursing home, or other supervised medical, nursing, or pient care facility th provided a practical knowledge of human body structure and sterile techniques and procedures; OR Graduion from an approved school of least 24 months durion. LPNs the GS-4 grade level perform all duties expected of employees the GS-3 grade level; however, they are expected to exercise greer judgment, require less supervision, and to opere in accordance with all established policies, procedure, and techniques. GS-5 Completion of least one year of qualifying experience the GS-4 level or equivalent. Demonstred knowledge and ability to provide a full range of practical nursing care to pients with a variety of physical and/or behavioral problems. Works with the RN and/or MD/DO to appropriely orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relion to acceptable standards of practice in promoting optimal pient care delivery. Recognizes and appropriely responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to approprie personnel for corrective action. Technically proficient in initiing, performing and completing assigned duties in providing care to variable pient populions. Knowledge and ability to appropriely carry out assigned pient care based on the pients’ conditions; to use judgment in selecting the approprie order and sequence of procedures and trements; and to accurely recognize, report and record relevant pient informion. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for approprieness and conformity with established policies/procedures. Ability to observe, identify and respond to the pient’s needs for care, including medicion, equipment-assisted care and pient/family educion. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-reled factors. Prepares and administers prescribed medicions (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs trements according to established policies/procedures.
