Licensed Practical Nurse (LPN) Description Join our dedicated team as aLicensed Practical Nurse (LPN) in our New Way Services (NWS) Division,where we provide critical care and support to individuals living withacquired brain injuries (ABI) or disabling medical conditions, such asmultiple sclerosis or severe diabetes. This role focuses on deliveringcompassionate care, ensuring comfort, dignity, and the highest qualityof life for each resident. New Way Services (NWS) offers a supportive,home-like environment for individuals facing significant healthchallenges. Through personalized care, skill-building, and communityintegration, our team helps residents achieve greater independence andquality of life. Programs like The Resource Center provide additionalopportunities for engagement, self-expression, and goal setting,ensuring that each individual’s journey is as fulfilling as possible.
As an LPN, you’ll play a key role in supporting residents across 3-4residential group homes. This role is ideal for nurses seekingmeaningful work, a positive work-life balance, and a supportiveenvironment.
What You’ll Do:
Provide triage and nursing support forresidents, ensuring timely responses to medical needs. Conduct regularvisits to group homes for health assessments, staff support, andparticipant education. Monitor and manage health conditions, such asdiabetes, while coordinating care plans. Complete medication audits andensure compliance with MAP policies. Accompany or arrange participanttransport for medical appointments.
