The Department of Developmental Services(DDS) seeks compassionate nursing professionals to apply for our
LICENSED PRACTICAL NURSE II
(LPN II) positions. This is an excellentnursing opportunity for potential candidates who are interested infurthering their career and broadening their nursing experience toindividuals with intellectual and developmental disabilities. The LPN IIis the second level in the series and candidates will deliver care toour individuals living in a community residential home setting. Our LPNsare an extremely important part of our team, developing relationshipsand working with, not only our individuals, but family members,significant others, provider agencies and other appropriate parties inorder to improve the overall health/quality of living for thosesupported by the agency.

Agawam |Amherst | Athol | Baldwinville | Barre | Belchertown | Bellingham| Charlton | Clinton | East Longmeadow | Easthampton | FeedingHills | Gardner | Granby | Greenfield | Hadley | Hampden |Hatfield | Holden | Hubbardston | Lee | Milford | Monson | NorthBrookfield | Orange | Palmer | Phillipston | Shrewsbury | SouthBarre | South Hadley | Southampton | Southbridge | Springfield |Stockbridge | Sturbridge | Templeton | Uxbridge | Ware | Warren |Webster | West Springfield | Westfield | Westhampton | Whately |Wilbraham | Worcester Duties and Responsibilities (these duties are ageneral summary and not all inclusive): Provide exceptional nursing careand oversight by implementing the individuals’ health care planincluding charting vital signs, application of medical/surgicaldressings, and administering medications as prescribed and directed bythe primary/specialty physician Assist with daily maintenance, personalcare and other services of support as outlined in the Individual ServicePlan (ISP); Maintain individual records and files Initiate referrals forneeded medical services including scheduling, hospital safeguarding,advocacy and other follow-up care; Engage in referral for supportive andhealth-related protections Communicate health issues to staff and updatethe individuals’ families/guardians in matters includinghospitalizations, rehabilitation support such as physical/occupationaltherapy Inform staff of any medical concerns related to activity levelthat may hinder and individual’s progress Order medications andmaintain medical supplies, materials and equipment Initiate emergencymedical procedures by applying basic first aid techniques and/or CPR andtracheotomy care, and catheterizations; Provide training for directsupport staff on use of equipment and treatments Transport individualsto work, medical appointments, recreational activities and othercommunity engagements as needed
Position Requirements:
Must possess acurrent, active license to practice as an LPN in the Commonwealth ofMassachusetts.
Agency Requirements:
Must possess a current and validClass D Motor Vehicle Operator’s License Incumbents must obtain andmaintain Health Care Provider Basic Life Support (BLS) certification
