Job Overview The LPN provides nursing care to patients andfamilies in their home . The scope of nursing includes directpatient care, and patient advocacy in the coordination of care withthe multidisciplinary team, and with the nursing case manager. TheLPN may collect data on the health status of the patient, inconjunction with other members of the health care team, for theidentification of health goals. The LPN assures care for patientand families that is reflective of the organization’s core values.
The Licensed Practical Nurse is knowledgeable about and supportiveof the primary nursing model.

Duties and Responsibilities :
The duties andresponsibilities listed below are intended to describe the generalnature of work and are not intended to be an all-inclusive list .Other duties and responsibilities may be assigned. Assess, andusing evidence based practice, implements nursing interventionsAssess the patient’s physical and psychosocial needs to determinehealth status within the scope of licensure. Collects relevant dataand documents the information according to Medicare record policy ≺ocedure. Completes all nursing documentation per procedures andnursing standard of practice including assessments, interventions,responses to interventions, communications, verbal orders, etc.
Derives a holistic nursing diagnosis from comprehensive patientassessment and prioritizes same according to patient need . Nursingdiagnoses are documented according to agency record procedure andare reviewed and revised as indicated . Follows the nursing careplan, established by the RN Case Manager, based on nursingdiagnoses that includes measurable, realistic, and time directedgoals and nursing interventions specific to the problem andindividual patient; reports information about progress or changesto the RN Case Manager. Follows the nursing care plan demonstratingconsideration of the following criteria: Patient learning needs;Patient’s priority needs; Patient’s maximum health potential;Patient’s changing health status; Patient’s discharge planningneeds; Safe, skillful, effective and efficient therapeutic action;and Collaboration and communication with other health care teammembers (i.e., R.N.’s, rehabilitation staff, physicians, socialservices, community agencies staff and home health aides).Communicates the effectiveness of the nursing care plan as neededand consults with RN Case Manager regarding revision of the plan ofcare as needed. Provides therapeutic care, education and referral,seeking maximum patient & family participation and independence .Keeps the RN Case Manager informed of visit via voice mail messagesor in person after patient assignment completed. In consultationwith the RN Case Manager, may coordinate the delivery ofmultidisciplinary services to a primary patient population throughappropriate referral , follow-up, and advocacy. Assesses patientneeds relative to HHAs and communicates with team RNs and HHAManager. Demonstrates responsiveness to the observations andconcerns of the HHA; documents accordingly. Develops, maintains ,and applies a knowledge of third party payer (Medicare, Medicaidand, private insurance) guidelines governing eligibility forreimbursable home health services Documents data in thecomputerized patient record, and in a timely , legible, organized,concise manner to insure compliance with legal, certification, andthird party payer requirements. Seeks supervisory consultation withunusual/unfamiliar patient care situations and/or questionsregarding home health policy and procedure and/or reimbursementguidelines. Maintains a current body of knowledge and clinicalcompetence through ongoing participation in continuing educationrelevant to the area of practice. Participates in performanceevaluation through professional and clinical goal setting.
Understands and practices agency safety guidelines and proceduresincluding general safety, office (ergonomic) safety, fire safety,proper body mechanics, and proper patient/material handling.
Performs in a professional manner at all times, and maintainsprofessional boundaries. Assists in orientation and education ofnew employees and students as assigned; provides fielddemonstrations and acts as Preceptor . Demonstrates a knowledge andunderstanding of what to report to the CEO or Chief ComplianceOfficer when concerns of corporate compliance arise . Ensurescompliance within guidelines set forth by regulatory agencies (DPH,ERISA etc.) and demonstrates compliance with Home Health Foundationpolicies and procedures. Practices confidentiality principles setby the agency and federal

guidelines. Participates inall mandatory in-services. What we
