Summary We are seeking a RN or LPN – Admissions Nurse to join our team at Life Care Centers of America. In this role, you will be responsible for conducting patient assessments and facilitating the admissions process, ensuring that each resident receives the personalized care they deserve. This position is crucial in maintaining our commitment to excellence in patient care. Responsibilities Conduct thorough physical examinations and assessments for incoming patients. Coordinate the admissions process, ensuring all necessary documentation is completed accurately. Administer injections and manage medication as prescribed by physicians. Collaborate with interdisciplinary teams to develop individualized care plans. Provide education and support to patients and their families regarding care options. Maintain accurate records in compliance with healthcare regulations. Requirements Current (RN) (LPN) license in the state of practice. Experience in skilled nursing or assisted living environments preferred. Strong assessment and critical thinking skills. Excellent communication abilities to interact effectively with patients, families, and team members. Familiarity with eClinicalWorks or similar electronic health record systems is a plus.
