SUMMARY:

As a Licensed Practical Nurse, you will provideprofessional patient care to long-term and post-acute careresidents. You will direct and monitor the care provided by CNAsand consistently ensure proper documentation and recording istaking place. Your ability to lead is crucial in both the successof the CNAs and the care of the patients.

RESPONSIBILITIES
  • Responsible for timely, accurate medication administration ofassigned team and concurrent required documentation.
  • Performstreatments and documents progress of same as per physician ordersand facility protocol.
  • Assures concise, pertinent and completeresident care documentation using computerized medical recordprocess. Assures the initiation of appropriate care pathway tools.
  • Responsible for receiving/giving relevant reports, both writtenand verbal from/to appropriate staff as per departmental protocols.
  • Assures proper monitoring of residents and relevant conditionstatus are reported to charge nurse/supervisor and/or physicians ina timely manner.
  • Completes pre-reviews of medicationadministration records as per facility protocols.
  • Completesrequired MDS documentation and all related observations in a timelymanner.
  • Supports CNA team through assisting in direct ADL care ofresidents.
REQUIRED QUALIFICATIONS
  • Graduate from an accreditedSchool of Practical Nursing.
  • Current licensure in State of NewYork.
PREFERRED QUALIFICATIONS
  • One year experience preferred.

BLS

  • Basic Life Support
  • American Heart Association (AHA), LPN
  • Licensed Practical Nurse
  • New York State Education Department (NYSED)

  • Exerting 20 to 50 pounds of force occasionally,and/or 10 to 25 pounds of force frequently, and/or greater thannegligible up to 10 pounds of force constantly to move objects;Requires frequent walking, standing or squatting.
