Job Summary The Licensed Practical Nurse (LPN) provides generalnursing care to patients, families and caregivers in the MedicalGroup practices. The Licensed Practical Nurse (LPN) assists andadvocates for the patient and families health in support of thepatient centered approach and delivery model of care, and is anactive care team member. Responsible for collaborating withphysicians/ providers , registered professional nurses andmultidisciplinary care team members. A Licensed Practical Nurse(LPN) can perform tasks and responsibilities within the frameworkof case finding, health coaching, and supportive care under thedirection of the registered professional nurse or licensedphysician or licensed health care provider. Nursing shall performin accordance with the standards outlined for Licensed PracticalNurse (LPN) in the NYS Education Law, NYS Office of theProfessions, and by the State Nurse Practice Act. A LicensedPractical Nurse will follow the mission, vision, I-CARE values,policies and procedures of Mohawk Valley Health System. Core JobResponsibilities Gather patient measurements and collect patientdata Record and report patient findings Prepare patient for patientexamination and treatment Assistswith examination and treatmentsPerform tasks within the framework of supportive care, under theclinical direction of licensed health care provider or registeredprofessional nurse Support self-care , self- management and theestablished plan of care. Utilize standard precautions and practiceat all times Maintain equipment, supplies, inventory, and a safeand clean work environment Maintain technical knowledge byattending educational workshops, mandatory in-services; reviewingpublications and participating in department committee meetings orwork groups Engage in quality improvement initiatives includingcontinuing education, participation in system-wide and departmentspecific teams Maintain a cooperative relationship among thehealthcare team by effectively communicating information Respond topatient requests, communicates effectively to the licensed healthcare provider who can fulfill the patient request
Education/Experience Requirements Required:
Graduate of anaccredited school of licensed practical nursing program
Preferred:
Bachelors or Masters of Science in Nursing from accredited nursingprogram
Licensure/Certification Requirements Required:
