Mon – Fri8:00-4:30/ On call weekend commitment

Monthly/Quarterly Location:

Kansas City Area – Service area can include Leavenworth, Johnson,Wyandotte, Franklin, Douglas and Miami Counties in Kansas; Jackson,Platte, Clay, Cass Counties in Missouri The role you’ll contribute:

This position is accountable for providing professional nursingactivities as identified and planned by the R.N. Case Manager inthe Patient Care Plan to maximize function within the homerestoring health or assisting a peaceful death. The L.P.N. issupervised in the home by the R.N. as often as necessary, but atleast every sixty days. The value you’ll bring to the team:

Minimumqualifications:

Graduate of an approved school of nursing At leastone (1) year of nursing experience in a hospital setting orcommunity setting involving direct nursing care Current license topractice as a professional nurse in the State of Kansas andMissouri. Active American Heart Association BLS Valid Driverslicense Preferred qualifications: Previous home health experience.
