Schedule Options 4 days/week, 10-hour shifts 5 days/week, 8-hour shifts Monday start, no weekends required (unless on-call) What You’ll Do Provide skilled nursing care under RN supervision Administer medications and treatments as prescribed Monitor patient conditions and report changes to the RN or physician Educate patients and families on care plans and medications Document care accurately and timely using EMR Support patients in achieving their health goals at home What We’re Looking For Active LPN license in Missouri Minimum 1 year of clinical experience (home health preferred) Strong clinical and communication skills Ability to work independently and manage time effectively Reliable transportation and valid driver’s license Why Breakthrough Healthcare?
