Home Health LPN Full Time Job Summary:

Provides direct and indirect patient care services that meet the psychosocial, physical, and general aspects of care; meets the communication needs of patients and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the direct supervision of a Registered Nurse. Maintains regulatory requirements, Nursing and Home Health & Hospice policies, procedures, and standards. Communicates with physicians and team members about changes in patient’s clinical conditions or response to treatment. And additionally, is able to perform general nursing assessments with observation and instruction of the patient. LPN will be provided with adequate orientation and supervision to perform job duties and participate in Quality process improvement (PI) projects. A registered nurse (RN) will complete a nursing history and physical assessment of the patient admitted to MRHC Home Health. The LPN will perform direct patient care in the patient’s place of residence following the plan of care/485/orders as transcribed by the RN/MD. The LPN may take verbal and telephone orders from a licensed independent practitioner (physician or N.P.) or a physician’s licensed staff member to include RN, LPN, EMT, RT, PT, OT, ST, RPH, Pharm D within their scope of practice. LPN must use the “read back” process to verify information communicated. Education Requirements Graduate of an accredited program Nursing Required. Mississippi Board of Nursing or Compact State Licensure as Licensed Practical Nurse. Must maintain continuing education hours as required by state of residence licensure. Certification & Licensure Requirements CPR and IV certifications are required either at time of hire or within one year of being hired/or at the first available IV certification course offered by MRHC. Experience/Qualifications/ Skills Healthcare/Medical –

Nurse:
LPN, LVN

Preferred. Experience preferred but not required Language/Communication Skills English Required. Multi-Lingual capabilities enhance the ability to perform this job. Must have the ability to speak effectively and professionally to all staff, management, and all outside individuals at all levels from elementary to professional. Mathematical Skills Skill in adding, subtracting, multiplying, and dividing whole numbers, fractions, and decimals without a calculator. Skills in performing operations with units such as cup, pint, and quart; inch, foot, yard; ounce and pound. Knowledge of the metric system units (length, area, volume, capacity, mass and weight) and U.S. equivalents. Skill in computing ratios, rates and percentages. Reasoning Ability Must have the ability to apply non-judgmental reasoning capabilities whenever necessary when dealing with patients, family members and visitors in the home environment.

Critical thinking skills a must:

Must have reasonable reflective thinking focused on deciding what to believe or do. Must possess purposeful, self-regulatory judgment which results in interpretation, analysis, evaluation, and inference, as well as explanation of the evidential, conceptual, methodological, criteriological, or contextual considerations upon which that judgment is based.
