The ideal candidate will be responsible for providing high-quality patient care in various settings, including urgent care, home community care, and occupational health. This role requires a strong commitment to patient advocacy and the ability to work collaboratively with healthcare professionals to ensure optimal patient outcomes. Duties Administer medications and treatments as prescribed by physicians. Provide case management support, coordinating care plans for patients. Assist in sterile processing and maintain a clean and safe environment. Monitor patients’ vital signs and report any changes to the nursing staff or physicians. Engage in patient education regarding health management and wellness practices. Support urgent care initiatives by responding promptly to patient needs. Collaborate with interdisciplinary teams to enhance patient care delivery. Facility Marketing / Review and assess hs patients Qualifications Current LPN license in good standing. Proven experience in patient care, preferably in urgent care or home community care settings. Knowledge of case management principles and practices. Familiarity with sonography techniques is a plus. Strong communication skills and the ability to work effectively with diverse populations. Ability to perform physical examinations and understand occupational health standards.
