Licensed Practical Nurse, LPN Position Summary:

Licensed Practical Nurse, LPN, Full-Time or Part-TimeAvailability LPN provide daily support to the unit leadership & respondappropriately/timely to resident/family needs within the LPN scopeof practice LPN will use nursing process, judgment, & skills to setpriorities necessary to deliver quality direct resident care & makedecisions promoting the safety/quality direct care toresident’s LPN perform/administer standard nursing treatments and/ormedications & observes resident for desired or unwantedeffects LPN communicates & collaborate with appropriate member(s)within the care team to revise/update the Nursing Care Plan or anypertinent findings including lab/test results, incidents, &/orconcerns LPN monitors & documents vitals, treatments administered, &residents’ responses Prepare, organize, oversee, monitor, & evaluate delivery ofcare of resident CNA assignments & activities, considering thecategory of needs & competency of personnelPutnam Nursing ℜhabilitation Center, is a 160 bed skilled nursing home andrehabilitation facility located in Putnam County, New York.
