Overview Job Summary Provides patient care as directed by the professional registered nurse and is accountable for own actions under this direction. Provides a caring relationship that facilitates health and healing and delivers care in a manner that preserves and protects patient autonomy, dignity and rights. Provides direct care focused on rheumatic diseases, under the supervision of registered nurses or physicians. Demonstrates professional appearance and behavior, and ability to work in a group. Performs other duties as assigned.

Responsibilities Patient Assessment and Care:

+ Gather patient health information, monitor vital signs, document observations. and report abnormal findings. + Assess patients’ symptoms, medical history, and overall health status. + Review Rapid 3 Scoring Tool & Homunculus for change in health status. + Report patient health status changes to provider and document in chart.

Medication Management:

+ Reconcile medications, order medications and administer as prescribed by provider. + Monitor for side effects according to the American College of Rheumatology. + Obtain knowledge of Rheumatologic medications commonly used and common side effects. + Communicate procedures regarding set and delivery of specialty medications. + Follow up with patient regarding set up and deliver of specialty medications.

Patient Education:

+ Provide basic education and counseling to patients about their conditions and management strategies. + Document patient education and provide written materials and/or appropriate website education available. + Communicate with provider regarding patient questions about treatment options.

Procedure Assistance:

+ Assist with procedures such as joint injections, blood draws, and wound care under the direction of physician or licensed provider.

Communication:

+ Answer patient phone calls, patient advice requests and messages in Epic. + Provide patient information under the direction of physician/nurse. + Evaluate patient condition and report findings to physician/nurse.

Documentation:

+ Maintain accurate records of patient care and treatment. + Document all interactions with patient and providers in Epic as indicated by policy.

Safety and Hygiene:

+ Ensure a safe and clean environment for patients. + Clean rooms in between patients as required by JCAHO.

Professional Development:

+ Participates in activities designed to improve health care delivery. + Participates in ongoing educational activities. + Completes assigned goals as assigned by manager/provider. + BLS certification

Experience:

Previous Nursing Experience. Previous uniterupted work experience in a healthcare setting.

Minimum Required:

Current LPN Licensure in Mississippi.

Preferred/Desired:

Previous Rheumatology and/or Internal Medicine clinic experience.

Phlebotomy Education:

LPN training at accredited institution. Minimum Required + Completion of accredited LPN program Preferred/Desired Training-Phlebotomy; Medication Administration; Referrals; Electronic Health Care Documentation; Phone Etiquette. Minimum Required Preferred/Desired Special Skills-Phlebotomy Minimum Required + Computer literacy Preferred/Desired Licensure Minimum Required + BLS
