HOSPICE LICENSED PRACTICAL NURSE

HOSPICE LICENSED PRACTICAL NURSE

(LPN) Part Time(Burlington/Durham)

Job Description:

• Provides direct patientcare, to include using a variety of skills and necessarycoordination of services.
• Completes appropriate documentation asrequired by policy and procedure and regulation.
• Follows the planof care appropriate to the patient’s diagnosis and condition asdeveloped and assigned by the Registered Nurse.
• CoordinatesHospice services with the Registered Nurse, Patient CareCoordinator and other agency personnel to optimize patient care, aswell as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwardsall clinical, progress, and summary notes to the Hospice RegisteredNurse for coordination of care.
• Attends and participates in IDTmeetings.

Job Requirements:

• Successful completion of nursingeducation from an approved school of nursing.
• Current LPNlicensure in the state in which the branch is located and one yearof clinical nursing experience, preferably in an acute caremedical/surgical setting.
• Ability to independently organize workand provide effective health care services to hospice patients.
• Ability to ensure appropriate patient care as supervised by theHospice Registered Nurse, and document thoroughly, accurately, andcompletely on a timely basis.
• Effective communication skills(oral and written).
• CPR certification.
