• Roles and Responsibilities:

• Patient Care:

  Administer medications, monitor vitals, and provide direct nursing care.

Assessment:

Assist in care plan development and conduct ongoing patient assessments.

Documentation:

Maintain accurate patient records.

Communication:

Effectively communicate with patients, families, and healthcare team members.

Collaboration:

Work with interdisciplinary teams to ensure quality care.

Compliance:

Follow facility policies, infection control, and safety protocols.

Education:

Educate patients and families on health issues and self-care.

• Requirements:

• Experience:

  1 year of recent LPN experience in LTC.

Education:

Accredited LPN program graduate.

Licensure:

NC or compact LPN license.

Certification:

BLS required.

Skills:

Strong clinical, communication, and organizational skills; PCC experience preferred.

Job Types:
