Licensed Practical Nurse – The Charles George VAMC currently has one (1) opening in Acute Care on the Specialized Work Assignment Team. The LPN provides effective educion to pients and or family members in relion to disease process, medicion and or prescribed trement regimes. The LPN demonstres active collaborion and communicion with PACT (Pient, Primary Care Provider, RN Care Coordinor and Medical Support Assistant) and larger team (family/caregiver, internal and community-based services and disciplines involved in providing care to the pient). Duties of this position include but are not limited to the following: Technically proficient in initiing, performing, and completing assigned duties in providing care to variable pient populions. Knowledge and ability to appropriely carry out assigned pient care based on the pents’ conditions; to use judgment in selecting the approprie order and sequence of procedures and trements; and to accurely recognize, report and record relevant pient informion. Completed work should need only a general review by a registered nurse or physician for approprieness and conformity with established policies/procedures.
