Duties:
1. Performs populion health oversight to improve clinical outcomes of Primary Care panels to include
ASCS, VSSC
metrics and others as needed. 2. The LPN responsible for ensuring chart audits are completed monthly and da is compiled and reported to leadership. 3. Serves as a resource for complex/specialized procedures (i.e. noncomplex suture removal noncomplex wound care, and clinic specific procedures, such as ear irrigion and bladder scans). 4. The LPN will serve as a Subject Mter Expert (SME) for clinical reminder educion and completion 5. See number 8 — orient all new LPNs on their role in populion health. 6. Demonstres knowledge of standard precautions, medical asepsis, and sterile technique with the ability to correctly apply these principles and techniques and assist others with implemention in the clinic setting. Assists with diagnostic procedures th include preparing the pient, setting up sterile field and instrumention, and monitoring the pient’s condition. 7. Assists with the facilition of HRO Huddles in conjunction with leadership. 8. Serves as a formal and informal preceptor/mentor for all new LPN/HT (and RN when needed) hires in the clinic. Participes in the interview process for selection for new staff hires when requested. Completes Preceptor Mentor Coach (PMC) training. 9. Demonstres self-direction by identifying and initiing annual laborory testing needs for certain pient populions, such as annual HgA1c, LDL, and diabetic nephrophy tests for pients with diabetes, hypertension, and coronary artery disease. 10. Collabores with NCM RN to ensure th pients who left the Medical Center without being seen or against medical advice are contacted in a timely manner to address potential care needs. 11. Collabores with PACT team to identify and proactively manage high- risk pients through frequent telephone contacts and/or face-to-face s using the LPN clinic grid as when indiced. Performs and documents pient telephone contacts for monitoring pients who recently discharged from the hospital within 48 hours of discharge (will we ask this to be done in 48 hrs). 12. Uses self-direction in clinic intake of scheduled and walk-in pients and identifies potential clinical problems or deviions from normal. Uses sound judgement to determine communice and initie action as approprie. 13. Maintains and implements knowledge of anomy and physiology as well as scientific principles underlying nursing care and procedures. Provides high quality nursing care using traditional and non-traditional modalities when approprie, including triage prior to provider face-to-face s, nurse health maintenance s, telephone triage, secure messaging, and Clinical Message Relay Queue (CMRQ). Applies knowledge by considering the interrelionship between age, physical, emotional, social, cultural, and populion-specific needs when providing care.

GS-7 . The GS-7 grade level is for select, complex LPN/LVN positions, as established by each health care facility. To be advanced, employees must meet all of the following standards, provided the standard is part of their assignment.

Such individuals may also serve as leaders or co-leaders in a nursing unit or for a specifically identified group of pients. (a) Completion of least one (1) year of additional qualifying experience the GS-6 level or equivalent, fully meeting all of the performance requirements for the
GS-6 LPN/LVN.
(b) Knowledge and skills necessary to carry out more specialized pient care duties, including assignments in selected, more complex inpient and/or outpient care settings. Knowledge base promotes the incorporion of current pient care da in actively contributing to the development of the pient’s overall health plan and in carrying out assigned nursing interventions. Positions this grade level require broader knowledge and skills because supervision is more general and employees are expected to be involved to a greer degree in the development and modificion of trement plans. (c) Ability to assist RN and/or MD/DO to appropriely prioritize overall pient care needs and adjust plans for care delivery. Makes astute observions of subtle, less obvious changes, both physical and emotional, in pient’s condition, and accurely and thoroughly documents care delivered and pient’s response to care th could require immedie modificion of the pient’s care plan. Appropriely seeks assistance and guidance by informing RN or MD/DO of changes in pient’s condition requiring higher-level intervention. At this grade level, the LPN/LVN is expected to participe to a greer degree in prioritizing, and, as approprie, modifying pient care trement. They must also possess the ability to observe more subtle physical and emotional changes in pients and demonstre taking approprie action based on those observions. (d) Ability to proactively assist the RN or MD/DO in addressing pient’s needs for medicion/trements, accurely administering prescribed medicions/trements according to established policies/procedures and appropriely recording administrion and pient’s response to medicions/trements received. (This may include restricted medicion locally authorized for administrion by employees this grade level.) Demonstres the knowledge and ability to readily recognize the more subtle signs and symptoms of potential or actual drug reactions, promptly inform the RN or MD/DO of observed changes in pient’s condition and initie approprie, prescribed interventions. Employees this grade level demonstre broader knowledge and skill with respect to these responsibilities than employees lower grade levels. (e) Knowledge and skill to promptly recognize potential urgent or emergent pient care situions, seek assistance of the RN and/or MD/DO for approprie team intervention and effectively intervene, as directed, to assure the continuion of optimal, safe and therapeutic pient care delivery. At this grade level, the LPN/LVN must have a broader knowledge of the pient trement process and exercise judgment th demonstres the ability to effectively intervene when approprie. (f) Assists individuals or groups of pients and their families to take an active role in promoting healthy lifestyles for more positive pient outcomes. Exercises skill in influencing and communicing with unusually difficult to care for or communice with pients and/or family members who may exhibit problems with lack of self-control, resistant or abusive behaviors, or impediments in their ability.
