Purpose of the

Position:

The Hospice LPN plays avital role in delivering compassionate, high-quality care topatients at the end of life. Working under the direct supervisionof a registered nurse, the Hospice LPN is responsible forimplementing the plan of care, providing hands-on nursing support,and documenting all services accurately and promptly. The HospiceLPN collaborates closely with the interdisciplinary team to ensurethe physical, emotional, and spiritual needs of both patients andfamilies are met with excellence and dignity. This role requireskeen assessment skills, attention to detail, and a deep commitmentto patient-centered care. The Hospice LPN also ensures effectivecommunication with the supervising RN and all care disciplines,promoting continuity and quality of care throughout the hospicejourney.

Provide high quality patient care consistent with the patient’splan of care, agency policies and procedures, andcoverage/reimbursement guidelines, consistent with the NursePractice Act in the state where care is given. This includes directtreatment; reinforce teaching, and follow-up care to patients asassigned by supervising RN. Prepares clinical visit notes and follows patient care plans asdeveloped by the registered nurse and according to agencypolicy. Documents and ensures appropriate use of medications, DME andsupplies Provides education to patient, families and/or others in caretechniques specific for patient Reports any significant change or adverse reaction to treatmentto primary nurse; modifies plan as ordered by the physician Consults with the primary nurse regarding needs for additionalservices Attends educational programs to learn and review informationnecessary to provide patients with quality care Participates in IDG meetings Maintains confidentiality in all aspects of the job Assesses signs and symptoms indicating physiologic andpsychosocial changes in the patient’s condition. Communicates consistently and appropriately with patients,families, physicians, appropriate health professionals and teammembers. Promotes and maintains patient quality of life under theguidance of the registered nurse. Participate in department or organizational Quality Improvementactivities. Participates in on-call rotation. Willing to travel to Hospice coverage areas for caredelivery.

Qualifications:

Graduate of an accredited school of Nursing, current LPNlicense required, one year of clinical nursing experience, preferhome health or hospice experience Strong organizational, communication, listening, and assessmentskills CPR certification required Tb Skin Test within the last year Knowledge of hospice guidelines and be able to read andinterpret guidelines, policies and procedures Professional phone and interpersonal behavior; Respectconfidentiality of patient data Understanding of medical terminology and calculations Minimum of eight (8) hours of continuing educationannually Reliable transportation

Physical Demands:

Sitting for variouslengths of time while operating the computer with frequentstretching, talking, hearing, smiling, bending, and reaching. Theability to stoop or kneel, bends, lift, push, reach with hands/arm,and smell when providing patient care. Must be able to walk upstairs in patient residences. Requires moving supplies and/orequipment from vehicle to patient’s residence.
