RN/LPN Full Time 32 hours a week 11-7 $3000 Sign onBonus RiverWoods Durham is currently seeking a Full Time 32 hour aweek RN or LPN to provide direct resident care in a team-focusedenvironment where our residents are at the heart of what we do.

Tuition ReimbursementResponsibilities:
Performs professional duties in accordance withfacility policies/procedures and standards of professionalpractice, State of New Hampshire Nurse Practice Act, OBRA andHIPAA. Collaborates with interdisciplinary team members toformulate individualized resident care plans. Updates care plans asnecessary. Participates in Resident Care Plan Meetings.
Institutescare protocols as indicated including:
falls prevention, safety andelopement, behavior management, weight management, skin integrity,bowel/bladder management, infection control guidelines (StandardPrecautions), and pain assessment and management. Assesses anddocuments changes in residents’ physical or mental status andreports findings to the primary care physician and treatment team.
Provides complete clinical data to facilitate decision-making.
Provides leadership and direction for direct caregivers. Preparesdaily assignments and delegates workload appropriately. Supervisesdirect care through observation and evaluation of direct careresults. Establishes positive working relationships with teammembers, residents and family members in order to facilitate open,direct communication. Obtains, administers and documentsmedications and treatments given to residents followingprofessional guidelines and Nursing department policies.
Inventories and maintains locked system for narcotics. Supports,creates, and maintains a positive culture for team members andresidents
Qualifications:
Must possess, as a minimum, a NursingDegree or Diploma from an accredited college, University or nursingschool. Long term care facility and specialized knowledge ofnursing care of the elderly and rehabilitative nursing ispreferred. Must hold a registered nursing (RN, LPN) license withinthe State of New Hampshire. Certified in intravenous administrationpreferred. Must maintain CEU’s annually. CPR is required.
