LPN – Primary Care 3.4 3.4 out of 5 stars 2500 Summit Avenue, Greensboro, NC 27405 AuthoraCare Collective is currently seeking an LPN (Primary Care) supporting our High Point, Kernersville and Winston-Salem service areas. This is a Full-time hybrid position : Monday – Friday 8:00am – 5:00pm. We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our primary care team. This position is primarily responsible for telephonic patient case management within the Chronic Care Management (CCM)/ Principal Care Management (PCM) scope. The LPN collaborates with physicians, nurse practitioners, and interdisciplinary team members to provide high-quality, patient-centered care. Strong clinical nursing and patient assessment skills. Excellent verbal, written, and interpersonal communication. Critical thinking, problem-solving, and organizational skills. Proficiency with electronic medical records and related software. Ability to manage sensitive information with professionalism and confidentiality. This role requires sound clinical judgment, strong communication skills, and the ability to support patients with complex medical needs. Provide telephonic care and case management to patients, ensuring timely, compassionate service. Enroll eligible patients in CCM and document appropriately in the EMR. Educate patients on chronic disease management under provider guidance. Follow up on labs, diagnostics, and patient inquiries per provider instructions. Deliver medication education and promote adherence; maintain an up-to-date medication profile in the EMR. Use the nursing process to assess, plan, implement, and evaluate patient care. Respond appropriately to patient emergencies and escalate concerns to an RN when changes in the Plan of Care are needed. Uphold safety, infection control, and HIPAA standards. Education and Experience :
Must be a graduate of an accredited LPN program. Must have a current North Carolina LPN license. Experience in primary care, case management, or chronic care preferred.

Valid state-issued driver’s license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
